Germany's approach:


Germany did not set up border limitations or common health status checks at airports so far. Instead - while in the containment stage - health authorities are focusing on identifying contact persons which are set to personal quarantine and are monitored and tested. Personal quarantine is overseen by the local health agencies. By doing so, authorities try to keep infection chains short leading to curtailed clusters. In protection stage the strategy will change to protect vulnerable persons from getting infected by direct measures. The mitigation stage will eventually try to avoid spikes of intensive treatments in order to upkeep the medical services.

639 new cases. They haven't said anything about infecting with intent but it seems we're following their lead.

No signs of school closing, we just have to get the kids to actually wash their hands, pretty sure they had an assembly to show them how:|. In better news though, classrooms all now have soap in them by the sinks and there is even hushed talk that the Y2 boys toilet sinks might be fixed as only one out of four has worked for about a year now. There has however been an acorn on the staff toilet floor for over a week now, which shows just how well they're cleaned each evening.

We've banned parents from attending any events in school for the foreseeable future, so prayers, masses, assemblies, our Easter Liturgy and our cafod 'mad' sales are all kids and staff only. Parents Evening is cancelled. From a selfish perspective, this is all a bit of a bastard with SATs approaching.

Plus the hand washing thing, of course.

Not the same situation as you, but I've had to put a contingency plan in place for my kids for if and when schools do close. They'll love me when they see the timetable I've drawn up for them...(not that many/any of them will stick to it). Also, leading teaching resource websites like twinkl and testbase are gearing up for home schooling.

As for bog roll, a pharmacy between here and work literally had piles of it going up to their roof when I drove past yesterday. I think they were taking the piss.

We do our 'big shop' on a Friday. It was heaving today and plenty of shelves were empty. Tomorrow will be manic; I hate food shopping at the weekend and it reminds me how much I detest people in general.

I went to a big Sainsburys after dinner and they seemed to be stocking the toilet paper back up (slowly).

I got 3 packs of 9. There were a few more of the 9s and quite a lot of the 4s though.

I also live right next to a Tesco express and every time I went in there last week they had stacks though so shouldn't have to worry.

I mean, it makes some sort of sense when you consider the Chinese model of locking shit down has nowhere to move because, if they open for business, they're open to more infections again. More nations are reporting cases and the world likes to travel. France, Germany and Spain are on our doorstep and they've not saw fit to close borders. The smaller nations around them have.

You need to be on top of your shit when it comes to monitoring, though. And you really, really have to keep on top of the mortality rate.

Having got pasta and rice slowly up to the max a couple of weeks ago, I've been going in on things like tinned fruit (which is weirdly untouched everywhere), peanut butter, Nutella, porridge, dried apricots, filled pasta (good best before on that) and, of course, cake.

Frozen pizzas also in the freezer to capacity.

I've had a dry persistent cough since Monday and shortness of breath and chest pains since Wednesday. I tried to get a doctor's appointment this morning as I rather miss being able to breathe freely but they of course didn't want me anywhere near the place and sent me to 111.

After about 4hrs (across 3 calls) on hold with 111 they sent me to A&E. A doctor met us outside and took me an isolation room and in the space of 30 minutes did all the standard obs, took a shitload of blood, did the Covid19 test (which is unpleasant) and a chest x-ray. Waited 2 hours for the results of the blood test to confirm I have a viral infection. Now I have to wait 3 days to find out if it's Covid19 and isolate for 7 days.

They couldn't do anything to help with the breathing though.

I'm back from my trip, with some observations:

Firstly, I got everything I wanted, bar the toilet roll. When I saw there wasn't any I wasn't going to bother with anything, but then it hit me, if I catch this cunt then I'm potentially not going to be able to leave the house for anything from 2 weeks+ without the risk of infecting others - I know the government want me to do that, but I'd rather not and even if I could guarantee I won't am I really going to want to get out if I'm feeling fucking awful. So back in I went perusing all the isles with one of the smaller trolleys. They were restocking furiously, so this meant much of what I wanted was available, albeit not some of the usual brands.

While toilet roll and eggs (no, I don't know either) were all gone, Nurofen was back on the shelves and I was also able to get some pasta, chopped tomatoes and all the tinned goods. There was one brand of pasta that was strangely still all there. Thinking it must be coeliac shit to start with, I initially dismissed it, but then the words MADE IN ITALY jumped out at me and that explained that. Picked 5 bags up (the max I could as per their polite request) and this theme repeated elsewhere. Chopped tomatoes. Pesto. Anything with made in Italy on the front is untouched.

After doing a few circuits I ended up with a trolley full of stuff that eventually amounted to just under £200, but that included toiletries, and household products. Up to this point it had been plain sailing, the place was reasonably dead and easy to navigate, bar the shop floor lads who were moving cages and unpacking them at an impressive rate. Then I got to the counter and as all the actual tills were closed I had to use a self service one, which for that much shopping was a right fucking ball ache. The technology on these things is now so outdated they make the McDonald's touch screen machines (before they patched them) look good.

I know some of you may think that much stuff is excessive, but I don't consider it stockpiling. I've just bought some non-perishable items, 2-4 week's worth, that will comfortably see me through should I catch it and be ill at home on my own. My Mum is in her mid 60s now so there will come a time in the not too distant future where I'd rather she didn't leave the house for a bit and most of my other friends and relatives round here have kids and I don't want to burden them with helping me unless I'm seriously ill. This food will be kept only to be eaten if and when I catch it and I'd suggest anyone living on their own do likewise.

Actual stockpiling may come later, depending on how it develops in other countries that are ahead of us in the spread of this fucker.

Oh and this may have been mentioned already here and I'm just regurgitating it without realising. Apologies if so, I've absorbed more information in the last 36 hours than any time I can remember, so keeping track of what I heard where is hard.

But, if herd immunity only works if 60%+ of the herd have it, we're going to need to have over 39m infected people for that to be the case, which in turn will lead to somewhere in the region of 750,000 to 1.5m people dying, many of which in a much shorter space of time than any other country will experience. For this to stand any chance of working the government either need to have a plan that is so Machiavellian and brilliant that they've factored the inevitable early doors rebellion into it or come clean with people about the longer term impact they believe will happen if we do what every other country is doing and get buy in.

I can't see how either of those options feasibly work.

I don't think it's that pointless. Anybody who is told they have a high temperature then goes for a actual test, self-isolates, or submits to a quarantine center. It seems to be a quick and easy way to possibly detect it. The lack of spread here would also indicate that it is working (amongst a lot of other factors).

They swab the inside of your mouth and both nostrils and the doctor who did it to me said the guidelines are to stick it far enough up your nose to produce tears.

Without wanting to spoil the fun, though, isn't the mortality rate more driven by relative availability of appropriate healthcare than, necessarily, by the deadliness of the illness itself?

Personally I think they should have used that study Kiko linked to where if you take total deaths to date, and divide them by how many cases we knew about 3 months ago you get a mortality rate well in excess of 100%.

Nothing is happening. Flights, if you can get one, are still moving freely. You could argue it would be pointless to do anything at this stage, but the peak is certainly not being suppressed as I can see.

Indeed, and I'm not saying our approach is necessarily wrong, for this very reason, but lockdowns will stop cases spreading, at least in the short term, which will be perceived as a success by the vast majority of people.

If we're then sat there, in it for the long haul, with deaths and cases still rising exponentially, while looking abroad at measures which are seemingly working, we're going to have civil un-rest on a scale of medieval proportions.

Yeah, of course. But what happens in between that time. Our cases are rocketing and everywhere else (bar the US, they are utterly fucked) are going down.

And these conclusions are fine to be drawn, the government have told us what they're trying to do, albeit not in so many words, so we're going to get a lot of dead and people can't handle that.They'll handle it even less well if its a disproportionate number for a few days, let alone the weeks/months this might take.

Thought I’d log back in as I might be able to provide some insight into how the Trust I work in is trying to manage this. And I’m going to need something to do if we enter full lockdown mode and one can only masturbate so much.

For context, my organisation has five hospital sites over a city and three towns in the Midlands. Two of those hospitals are acute sites and so will be taking Covid-19 patients.

So far the two acute sites have set up ‘cohort’ wards for patients with respiratory symptoms who require admission. We have already ring-fenced ICU capacity for Covid-19 patients only. I don’t know if those areas are full yet but suspect not as it’s something we’d likely be hearing about. What I do know is that the remaining ‘normal’ ICU capacity is full. Patients will move in and out as they do anyway but there is now clearly going to be very little tolerance in terms of capacity. The concerns about not having enough ICU capacity nationally are very real.

I’ve received word today that visiting patients is now done with immediate effect, other than in a small number of exceptional circumstances. My team (I’m a corporate programme wanker these days so further removed from the frontline than previously) will be meeting on Monday to discuss our contingency plans which are based on the existing national pandemic plan. Some will start working from home, some have clinical backgrounds so are likely at some point to be drafted into patient care, others like me have operational backgrounds and will probably be shifted into managing capacity and demand. But that’s me just reading between the lines for now.

I can’t imagine it will be long now before all routine outpatient appointments and surgery is scrapped. Everything will be moving away from scheduled stuff towards acute medical care. It’s just a matter of time. Like everybody else we are relying on the government’s plan being the right one. I have no particular insight as to whether it is or not. We do have consultants who are worried about it but they’re not epidemiologists. I doubt very much that even cancelling routine stuff is going to give us anything like enough capacity (that’s not just beds, it’s people, kit etc) to cope very well with the peak of this.

Depends what you mean by ‘coping’. As well as any health system which has been underfunded and understaffed against significant growth in activity. Not as well as it could be, but it isn’t about to collapse beyond usefulness and the government doesn’t have a dastardly plot to privatise it.

It is obviously going to struggle very badly through this crisis. We do have the advantage of being centrally controlled whereas in Lombardy there is some privatisation and thus fragmentation of resources and response. But in the context of years of shit funding that might end up being useless. We don’t know.

As for staying; dunno. Like most things I do and am not paid for it depends if it’s going to be fun or not.
