    Starting to see reports (from proper outlets) that pubs, restaurants and gyms in London will be told to close later. Maybe that was always the plan but they needed to sort the level of financial support for employees out first - which I think is also meant to be coming today.

    The nearby, how can I put it, minority-owned convenience store has loads of bread on the shelves, and a big box of eggs under the counter. I asked if they had any eggs, they sort of burbled to each other, and then one of them brings half a dozen up out of nowhere. Top lads. I bought an extra loaf and chucked it all to the swans on the way home as well because fuck the stockpilers. I might make that part of my new routine actually.

    It depends where London is against the peak. They've been talking about us being x week behind Italy but London by y weeks ahead of the rest of the country. So perhaps London is supposed to be on the verge of going full Italy.

    The other thing is that's a quote from one London hospital director. I've no reason to doubt it as the HSJ is almost always well informed. But Critical Care beds operate in regional networks. When I'm on call and my place runs out of ICU beds - which is rare-ish but always close enough to happening that you have to plan for it - then I sit down with whichever consultant is in charge and work out who we can most safely shift elsewhere in the network if it comes to it. The context is important. The director they've spoken to might think it's 'terrifying' because their ICU has filled up before the peak and they're already having to make decisions about transfer rather than London being completely fucked already. Both are shit, but one is less shit than the other.

    For me, there's the visualisation of what's going to happen and the numbers.

    All the posts I've made that are predicting doom are based solely around the numbers, but I still can't visualise day after day of news reports in this country opening with another x hundred have died, let alone thousands.

    So even I look at Italy and sometimes think "that won't happen here", and then I look at the numbers and look at what we're (not) doing again and it's inescapable that we won't be as bad, if not worse.

    And maybe that's the problem and why people aren't taking it seriously. They simply don't believe it will happen here, because they haven't seen it happen here and they can't visualise it.

    It's unlikely that we won't get into the hundreds per day. Whether that's Italy level stuff is a different question and there a good demographic and social reasons why it shouldn't get quite so bad here as it has there. Then again, they have double the number of ICU beds as we do so who knows? But we'll see.

    I'm not sure even posting those kinds of numbers per day will work, though. I wonder how long it takes for that to normalise. I mean yesterday another 30 odd died and I just thought 'thank fuck it isn't worse than that'. Yet if you told me some fucker had just wandered into a shopping centre and killed 30 in a one off event my reaction would have been quite different.

    On the societal reasons for Italy getting so bad, I get there are those, but isn't Northern Italy a very gentrified place vs. London with 7+ million people living and working on top of each other.

    The scientists must have factored that into the equation, surely.

    The real prepratory step would be to start publishing the non-Covid daily deaths alongside it for some perspective. Google says it's approximately 1,500. If the covid numbers are just largely reducing the 1,500 figure we can stop being such melts and let the fucking weak go.

    There are also loads of grannies and grandads living with the rest of the family in Italy which you don't get so much here. Not that London isn't fucked. The population density is going to do for it. I'm hoping me in my shit market town and the parents in their village might be okay as a result.

    That's actually not a bad point. It would be good to know just how far above the number of expected deaths on a given day this takes us. Also, the proportion of patients in hospital/ICU/dead are a bit useless without knowing how many people are wandering about asymptomatic or thinking they've got a bit of a cold/virus.

    Hence 'test, test, test. Which - fuck the vaccine - is what is going to allow us to start getting back to normal again.

    We will, but it will be months away. If we're a couple of months a way from 250k per day, and we're not too far off this antibody test, then it oughtn't be too long before we have a decent sample size. But it won't be before the bodies really start piling up either. The mortality rate is going to look fucking horrific for a while, but it won't be a true figure.

    The worrying thing about London is that so many younger people live there who have moved from other parts of the country and loads of them are going to flock back to their parents' houses and quite possibly take it all over the country with them.

    Yeah. A lot of the cases elsewhere in Italy were seeded by the fuckers who legged it out of Lombardy when they pre-announced lockdown.

    Fuck me. It feels like ages ago that this was just a shitstorm in a region of Italy after the Chinese had welded it in. This time last week I was still going to the pub tomorrow to watch Watford v Leicester.

Come the end of their 7 days they'll start bowling about thinking they're invincible.

    In paeds we aren't testing anyone unless they're admitted to hospital. I presume it's the same for the rest of the hospital (and the rest of the country).
