The last time Iran postured like this a few months ago, they fired a couple of shitty missiles and a pack of drones earmarked to get shot down over eastern Ukraine at irrelevant targets, just to show they'd done something.

The BBC are reporting that the dome is stopping "most" of the missiles, but from the videos all over twitter it looks like Israel is getting its shit rocked. If they're actually hitting buildings, rather than landing in the middle of nowhere - as James says - then it's hard to see this not resulting in a wider war and I would not want to be in Iran right now.

2) Israel had all the intel from the US well in advance and let it happen knowing it will be a damp squib. Israel hold all the cards in the region now and they will keep taking out Iran's proxies to weaken them into regime change.
I'll go with both 1) and 2).
I assume the whole process of missile interception allows you to see where the missile is likely to go, so any that do 'get through' are presumably of low priority in terms of being stopped. I would also assume that any significant military infrastructure probably comes with its own air defence system generally. Paired with everyone in the country being in secure shelters it's probably quite hard to inflict significant causalities with such ranged efforts, save by landing the debris on random mooks.
