Jos Hooiveld? Wikipedia says he's scored five so he's probably already been said.

He scored 7 for Southampton in the league, but they were all in the Championship the year they got promoted.

Some clues for the remaining players:

6 - (West Ham United) - Striker who unfortunately passed away at the age of 39. Was stabbed in the eye when 31 which forced his retirement. Also played for Crystal Palace and Southend (where he scored 25 goals in 2 seasons).

5 - (Bolton Wanderers) - Striker who scored a couple of very important goals for Bolton in the 1995 play-off final. Also had a spell at West Brom later in his career.

3 - (Manchester City) - Joined City in 1992 and made over 90 appearances. Centre-back who also played for Oldham and Sheffield United, where injuries hampered his playing time.

2 - (Derby County) - Spent 5 seasons at Port Vale as a first-team regular before joining Derby in 1995 for their promotion campaign. Also played for Wolves (on loan) and Barnsley. Retired at 32 and now manages Newcastle Town.

2 - (Manchester City) - Versatile player who could play at the back or in midfield - spent 7 seasons with City between 1997 and 2004, and was named their Player of the Year in their Division 2 promotion campaign of 98-99.

2 - (Derby County) - Striker who played for Ajax for 4 seasons (1989-1993) and finished his career off with 3 seasons with Derby (13 league goals in all).

1 - (Fulham) - Very obscure player, scored on his only Premier League appearance with a last minute equaliser against Crystal Palace in Fulham's last game before relegation in 2014. Flashy young midfielder who now plays in South Africa after spending time back in the Netherlands.

1 - (Swansea City) - Striker who had 2 spells on loan at Swansea before moving permanently. Never a prolific goalscorer, spent 6 seasons with Middlesbrough before moving to Wales. 14 goals in the Championship was his best season return (2011-12)

1 - (Southampton) - Defender who had one season (96-97) with Southampton. Made over 200 appearances for Feyenoord (in two spells) and also played for Galatasaray. Only league goal for the Saints came against Leicester.

1 - (Huddersfield Town) - International defender, began his career at Feyenoord before moving to Monaco. Signed for a club record fee after an initial loan spell for Huddersfield. Went out on loan in the Championship where he was unfortunately seriously injured this season.

1 - (Swindon Town) - Started his career at Haarlem, a defender who moved to Swindon from Motherwell. His only goal came in a draw against Manchester United, in March 1994.

1 - (Bolton Wanderers) - Midfielder who spent 2 seasons at Bolton (1994-96), before moving to West Brom where he quickly became a cult hero. Made over 250 appearances at the Hawthorns in 6 seasons, and fans would wear long blond wigs in his honour.

1 - (Swansea City) - A fullback who played for Feyenoord and Twente before moving to Swansea. Only made 27 league appearances in 3 years, he also had a loan spell with Middlesbrough and most recently played for Oxford United.

Dwight Tiendali for the Swansea full back. The rest I have no idea - dead West Ham striker has intrigued me though.

Tiendalli is correct. I'll leave the other 4 up for an hour or so to see if anyone can make a late guess before giving the names.

Correct! Jeroen Boere, who was stabbed whilst playing in Japan. Apparently he also owned a pub in Epping after his retirement.

Also, gormless, bald, French and United made me think of that Willie Prunier (I think) that was around in 95/96 - pretty sure he didn’t score, but it’s just odd to think that back then a triallist, not even a loaner, from a mid table Ligue 1 could play a handful of premier league games for the team that would go on to win the title. Bizarre.

One final clue for each of the remaining players:

4 - (Middlesbrough) - 2000-01 - Won the World Cup and the Champions League twice. Great hair.

4 - (Manchester City) - 2002-07 - Centre-back signed by Keegan, played briefly for Sheffield United after leaving City.

3 - (Wolverhampton Wanderers) - 2009-12 - Defender who moved from Marseille to Wolves, but found chances limited due to multiple injuries.

2 - (Nottingham Forest) - 1998-99 - Striker who had a season on loan at Forest fairly early in his career. Probably better known for a stint with Rangers - including missing a sitter and being sent off in the same Champions League game. Was eventually capped by French Guiana at the age of 37.

2 - (Watford) - 1999-2000 - Played for 16 clubs in his career, and earned 4 caps for France in the early 90s. Most successful during spells with Caen and Marseille. Scored his 2 PL goals in the same game v Southampton in December 1999.

1 - (Nottingham Forest) - 1997-99 - Very little information that I could find on this chap. A defender who played over 70 times for Forest in 3 years and also played for Auxerre and Lille.

1 - (Leeds United) - 2003-04 - 1 goal in a short loan spell for this imposing centre back who has since spent 13 years with PSG, firstly as a player and since as assistant manager. One cap for France, in 2001.

1 - (Sunderland) - 2001 - Spent time with Saint-Etienne either side of a very short spell with Sunderland. Has since spent over 10 years in management, including the Mali national side and various big names in African club football.

1 - (Fulham) - 2005-08 - Cultured centre-back who was supremely talented, but extremely injury prone, causing retirement at 30. Played for Barcelona, Marseille and Monaco before moving to Fulham under Chris Coleman. Sole goal was a last minute equaliser away at West Ham.

1 - (West Ham United) - 2002-03 - Spent a season on loan at West Ham in the middle of a 10 year spell with PSG. Midfielder who also went on loan to Monaco before moving to Besiktas and Marseille at the tail-end of his career.

1 - (Southampton) - 1998-2000 - Left-back whose only goal was a long-range effort against Charlton in early 1999. Spent 5 seasons at PSG in the early 90s, as well as time at Bordeaux and Marseille.

1 - (Hull City) - 2009-12 - Spent one season at Hull before moving to Wolves for approximately £2.5m. Ruptured his ligament and barely played before moving to Celtic for a season. Released at 27, and hasn't played since.

1 - (Bolton Wanderers) - 2009 - Centre-back who had a short spell on loan from Zenit. Also had 3 spells with Nancy and a notable 2 year loan with Monaco.

That goal against Wigan has to be up there with the hardest hit shots of all time, maybe alongside one of Tony Yeboah's efforts.

Dunne managed double figures, with 10 goals in the right end.

O'Donovan made 17 PL appearances for Sunderland, but no goals.

Whalley is, 1 goal for Bradford in early 2000. No caps for Ireland but played for the B team.

Time to bring back the trivia.

Mario Gomez played his last game (and scored) for Stuttgart this weekend. But can you name the full teams and used substitutes when Gomez made his Stuttgart debut, against Chelsea in March 2004?

Deco and Paulo Ferreira were winning the Champions League that season with Porto and Jose. Khedira was a couple years away from making his debut.

Remember the tie, was the round of 16 the year we eventually got done by Monaco. Chelsea would have been something like:

Cudicini (unless he was knackered by this point and Ambrosio had come in, but think that was a couple of weeks later)
Johnson
Desailly (possibly? his last season)
Terry
Babayaro (not sure about this one)
Geremi
Makelele
Verσn
Gronkjaer
Hasselbaink
Crespo

I think Mutu had blown out by this point.

As for Stuttgart, no idea. Hitzlsperger was the first name that came to mind.

Geremi, Mutu and Desailly were the 3 subs used for Chelsea. Stuttgart is tougher, but there are a few well-known names in their lineup.

Trying to think who our left back was in this era. Feels too late for Le Saux, but obviously well pre-Cole. Was it Bill Gallas out of position? That would be a Ranieri thing to do.

Mario Stanic sneaking in somewhere? Oh and Lampard obviously, how could I forget him.

Gallas played centre-back. 3 of the Chelsea starters left to get.

This left back is going to do my head in. I don't think Melchiot ever played there, or if he did it wouldn't have been in this game. Zenden never went back there for us and I think may have been out by this point anyway.

Was this the year we bungled in Jiri Jarosik in January? I think it was. Him or Scott Parker

Of course, should have remembered him from having donned the so-called invincibles in the next round. The most thrilling goal of my whole Chelsea-watching life, probably even better than Drogba's header vs Bayern due to the age I was.

I can't work out what formation this would have been if there was a midfield three. He usually played two up that season, so I guess we're missing a floating luxury player which must have been Joe Cole.

Hildebrand is correct, and I'd say there are at least 4 others that should be guessed. The rest are a little lesser-known.

Joe Cole made an appearance in the first leg from the bench, but was an unused sub in the second.

None of the above. Good shout with Tasci but a little before his time. Some clues for the remaining Stuttgart players:

CB - Brazilian who also played for Schalke as well as Stuttgart.
CB - Portuguese player who spent 6 years at Stuttgart, and scored the only goal of the tie - an own goal in the 1st leg that sent Chelsea through.
RB - 21 caps for Germany, played for Sevilla and Celtic also.
CM - Croatian veteran who played over 300 times for Stuttgart. Over 60 caps for Croatia too.
CM - Probably the most obscure of the lot - German who scored 27 goals in 8 years at Stuttgart as a midfielder.
LM - Better known as a full-back, this Croatian spent 6 years at Leverkusen between 1997 and 2003 and played briefly in England for Portsmouth.
Sub (ST) - Hungarian striker who played for a number of German clubs, as well as 2 spells with Sturm Graz in Austria.
Sub (CM) - Well-travelled German midfielder who played for Seattle Sounders later in his career. Over 540 appearances in club football before retiring last year.
